4-Year-Old Recovers from Complex Jaw Fracture with Minimally Invasive Surgery – Apex Hospitals

Mumbai: Apex Hospitals successfully treated a rare and complex pediatric mandibular fracture in a 4-year-old boy, using a minimally invasive approach in maxillofacial surgery. The child suffered a displaced condylar process fracture with temporomandibular joint (TMJ) dislocation after a fall while playing.

The surgical team, led by Dr. Vinod Pal, MDS (Oral & Maxillofacial Surgery), Fellowship in Head & Neck Oncology and Reconstructive Surgery, performed a closed reduction with intermaxillary fixation (IMF) using modified Gunning splints.

This conservative method avoids open surgery and preserves natural jaw growth. He was assisted by Dr. Gaurav Naik, MDS (Oral & Maxillofacial Surgery), Implantologist, with anesthesia managed by Dr. Sumeet Warang, MBBS, DA, Fellowship in Pediatric Pulmonology.

Imaging had shown an antero-medially displaced condylar process fracture, raising concerns about long-term facial asymmetry and jaw function. Customized pediatric Gunning splints were applied to stabilize the jaw for two weeks, followed by elastic guidance for one week.

Dr. Vinod Pal explained, “In pediatric patients, the condylar region is a critical growth center. Open surgery can affect future jaw development. Using modified Gunning splints, we can stabilize the jaw, restore occlusion, and protect growth, all without invasive procedures.

The child’s recovery has been smooth, with no facial asymmetry or functional issues.”

Post-surgery, the child followed a soft/liquid diet and began gentle physiotherapy after splint removal. Within weeks, he regained normal mouth opening and chewing function. A three-month follow-up confirmed full functional recovery and proper jaw alignment.
